Admission & Consent Forms

Thank you to everyone who has returned their child’s completed Admission & Consent forms to us. If you have not yet done so, please ensure the completed forms are with us as soon as possible so that we have the correct email contact details for you when your child starts in September. If your contact details are wrong then you will be unable to log in to many of our academy systems or receive academy communications about your child

Printed copies of the Admission & Consent Form may be collected from Reception, weekdays in term time between 8am – 3.45pm Monday to Thursday and 8am – 3.15pm on Fridays. You can download this editable Word version of the forms here and email it back to us:

Year 6 pupils who attended our two transition days in June were given a copy of our Coming to Westbourne Guide for 2025-26 to take home with them. It tells them what to expect when they join us in September:

If you have any questions or queries, please email wes-office@attrust.org.uk and somebody will usually respond in 48 hours. This might be longer during school holidays. The last day of term is Tuesday 22 July. Staff return to work on Monday 1st September for a PD day.
