IMPORTANT INFORMATION
Human Papillomavirus (HPV) Immunisation
The HPV vaccination helps protect boys and girls from HPV-related cancers. This virus increases the risk of developing some cancers later in life, such as cervical cancer, some mouth and throat cancers and some cancers of the anus and genital areas.
In line with the National Immunisation Programme, Hertfordshire Community NHS Trust’s School Aged Immunisation Service offers the HPV vaccination to all eligible boys and girls in Year 8 and in addition to this, those in Year 9 and 10 who have missed their vaccination due to the COVID pandemic will also be seen in school.
Your child will require two injections with the second injection given a minimum of six months after the first. In order to get the best protection, it is essential your child receives both vaccinations.
Please make sure on the day of immunisations your child is fit and well and encourage them to eat breakfast. Your child is advised to wear a short-sleeved shirt rather than a long-sleeved shirt.
We are aware your child may have already begun this immunisation programme (but not completed it). If this is the case, please continue to complete an online consent form anyway.
A response is required. You must accept or decline this vaccination by completing the online form. Please click on this link to go to the Immunisation Form and complete the form:
https://forms.hct.nhs.uk/HPVConsent/eForm/#!/Page1
You will need this unique code number EE139288 which identifies your child’s school.
The electronic link will close at midnight on Thursday 9 June. Please note the form will not work on some mobile phones and tablets.
It is important for you to discuss and complete the consent form with your child. Further information can be found online regarding the HPV vaccine, patient information and futher information about vaccines:
