Year 10 Meningitis ACWY, Tetanus, Diphtheria & Polio Vaccination Catch-up at Westbourne Academy

On Monday 19 July, the school nursing team will be at Westbourne to give catch-up vaccinations to students in Year 10 who missed their dose due to the partial school closures last year and again this year.

The MenACWY vaccine helps protect older teenagers and young adults from getting meningitis (inflammation of the lining of the brain) and septicaemia (blood poisoning). Tetanus, Diphtheria and Polio are serious diseases that can affect the nervous system, breathing and heart.

NHS records show that your child has not received these vaccinations, if you would like them to have them, please use the link below to give consent. To give consent – or decline consent – please follow the link below which will take you to an online consent form portal. The portal will close on 12 July 2021.

If you do not submit a consent form, we will contact you in order to confirm that you do not wish your child to receive the vaccine. If students are absent on the day of vaccinations, a further attempt will be made to offer the vaccine at a later date.

Please enter a valid mobile number and email address on the online form so that we can contact you if necessary.

THE FORM MUST BE SIGNED BY SOMEONE WITH LEGAL PARENTAL RESPONSIBILITY FOR THE YOUNG PERSON.

Please contact the immunisation team on: 0300 555 5055 or email: ccs-tr.suffolkimmsteam@nhs.net
